How to Sign Up

Please complete the following sign-up form to receive your complementary Great Place to Work survey, a $1,495 value.

Note that the survey covers the costs of administering your resident and family survey, which includes a high-level summary of overall results.

Upon submitting the form, you will receive a confirmation email. You will be introduced to your Program Manager by the next business day. Experienced working with home care providers, your Program Manager will guide your team through the survey process.

After your survey closes, you will receive your brand’s overall score, top 5 statements, top 5 areas of opportunity and notification if you have achieved Great Place to Work Certification. Finalists for the FORTUNE Best Workplaces in Aging Services list will be notified approximately 1 month prior to list publication. Survey completion is not a guarantee of Great Place to Work Certification, nor Best Workplaces in Aging Services list recognition. The first year of certification is assessed at no charge. Great Place to Work certification renews automatically on an annual basis, unless notified 60 days prior to renewal. The renewal date will be 12 months from the date of certification survey completion. Subsequent annual certifications will be assessed then-present certification rates.
